NC Medicaid

• Live in North Carolina

• Age 19 through 64 years old

• U.S. citizen or have an eligible immigration status

• Household income falls within the criteria

Residents can apply online, in person or by mail. The application requires documents and information about your household that you may need to gather.

Online

Apply online with ePASS. You may need to create an account first. On the application, NC Medicaid is referred to as “Medical Assistance.”

In Person

Apply in person at Sampson County Department of Social Services (DSS) office. You can also drop off an application at the Sampson County Department of Social Services.

Gather these documents with you:

• North Carolina residency
• Income Statement
• Birth Certificate
• Social Security Number
• Citizenship proof or Immigration status proof

It can take up to 45 days for the Department of Social Services to decide if you qualify for NC Medicaid. Disability applications can take up to 90 days. You will get a letter in the mail if you are approved or denied.
