Care Management for At-Risk Children (CMARC)

Services

  • Provides care management for Medicaid-eligible children and their families from birth up to five years of age.
  • To be eligible for the program, clients must have certain risk factors.
  • Requires a referral from an agency or medical provider.
  • Provides assistance with ensuring the infant/child and family receives assistance and referrals to community resources to assist the infant/child with achieving physical, developmental and mental age-appropriate milestones. 
  • Works with families in establishing a strong link to a medical home, other medical specialist, and provides growth and developmental screenings.

Case Management for High-Risk Pregnancies (CMHRP)

Services

  • Provides care management for Medicaid-eligible women from beginning of pregnancy through postpartum.
  • To be eligible for the program, clients must have certain risk factors.
  • Requires a Risk Assessment to be completed by a medical provider.
  • Provides assistance with pregnancy and referrals to community resources to assist the woman with having a healthy pregnancy.
  • Works with Pregnancy Medical Program (OB Doctors) to assist pregnant and postpartum women in improving healthier pregnancy outcomes.
  • Conduct comprehensive assessments to determine your strengths and needs, and work with you to meet those needs.
  • Provides education and offer referrals and linkages to other support services in the community.

Child Health Clinic

Services

  • Preventive health care for children ages birth to 21 years old: immunizations, check-ups, physical exams for child care, pre-k, kindergarten, elementary, middle, high school, and college.
  • Developmental assessments

  • Referrals for behavior services
  • Lead and Sickle Cell  screenings based on state recommendations
  • TB skin test for those from countries of high prevalence

  • Diabetes and Medical History Screening
  • Requires a referral from a medical provider.

  • Services for females and males
  • Family Planning: Including birth control pills, LARCs (Long-Acting Reversible Contraceptives), Depo-Provera, condoms, IUDs, and natural family planning
    • LARCS include: IUDs (Liletta, Mirena, Paragard) and implant (Nexplanon)

  • Female exams including pap smears, breast exams, etc., for women who may need a birth control method

Tuberculosis (TB) Control and Skin Testing

Services

  • TB skin testing
  • Chest x-ray referral for people with a documented positive skin test or bloodwork
  • Treatment, assessment and follow-up for suspected/confirmed TB cases.
  • TB screenings for work/school for people with a history of or a documented positive skin test and a negative chest x-ray
  • Treatment for latent tuberculosis infection (LTBI) and for TB disease
  • TB prevention education.
